1. Customer & Product Information

Please include your name, address, email, and phone number (including area code) with the item you are sending in, along with a detailed description of the problem.

Please fill out the warranty return form below and if possible include a copy with the product to be repaired. Once you have submitted this form please feel free to ship your item to us at your convenience.

If we find that the issue with your product is not covered under warranty, we will contact you with a quotation prior to performing the repair.
